 View and Print a Transcript Request Form |

| The Admissions Office must receive a completed form, signed by the individual making the request, before copies of a transcript are released. The postal mailing addresses for the Northwest-Shoals Admissions offices on both the Shoals and Phil Campbell campuses are printed on the form. Please complete and sign the form before returning it to the college. If you do not have Acrobat Reader software on your computer, you can go to the Adobe web site download and install the software free of charge. This software permits you to view and print all *.pdf files. Should you have questions regarding this process, email the Northwest-Shoals Admissions Office. | |
